PATIENT SUFFERS FROM DENGUE HEMORRHAGIC FEVER (DHF)

            One day  Vera   felt  headache and then she scream had off

 Vera               : achhhhuhh  my head throbbed painfully ,   any body help me ………

            After  Vera said something like that,  her younger sister drew near to  Vera and asked her  what’s her problem  

Rahayu            :  What happened  Vera ?

 Vera                           : achhhhuhh  my head throbbed painfully …….. ouch !!! that so pain . Rahayu help me please , I don’t want to die . Rahayu  do you know where is Mom?

Rahayu                        :  calm down  Vera …..   just know Mom went to the market for buying the    vegetable,  by the way have you  been having a launch  or are you having  problem that make you stress  then it makes you feel headache ?

 Vera               : no,  I don’t have problem that makes me stress until I have headache  and I  have  had a  lunch,

             Then,   Rahayu touched  Vera’s  forehead for ensuring that  Vera have a fever or not ,  Suddenly Rahayu saw red spots on  Vera’s the skin  

Rahayu            : Oh My Good   Vera  you have a fever …. What happen to your Skin ? Why   on your skin is  a lot of  red spots?

 Vera               :   I don’t know   Rahayu help me don’t want to die

Rahayu            : actually I don’t know what I have to do ,    I think it’s better we call Mom  by phone

 Vera                           : let’s call mom by phone quickly, because it’s really pain and it’s so inconveniently

            Then Rahayu Called her Mother for informing  that  Vera felt  her  head throbbed painfully (kring…… Kring……..) 

Mother            : hi honey what happen ? do you want me to buy something?

Rahayu            : no Mom,  I don’t want you to buy something,  but I’d like to inform you that  Vera feel  her  head throbbed painfully,  she has a fever and on her skin look a lot of  red spots.  I don’t know what I have to do …..

Mother            : what ? felt throbbed painfully, she have a fever and on her skin look a lot of  red spot how it’s happen ?

Rahayu            : I don’t know Mom , 

Mother            : Honey you have to bring  Vera to the  hospital immediately, because I am afraid she has serious disease , and I am going to go to hospital  letter , because the way on the market is really traffic jam.

Rahayu            : Yes Mom 

            After  her Mother said that, Rahayu immediately brought  Vera to the hospital by Taxi . because of  Vera  felt her head throbbed painfully, it makes she unable to walk properly.  So when  Rahayu Arrived  at Polyclinic of family Husada Hospital   Rahayu asked to the nurse    for helping  Vera walked to the examination room

Rahayu            : Nurse……  help me!!!!  to bring my younger  sister to examination room

Nurse  1           : Ohhh , What happen ???

Rahayu            : she   feel  her  head throbbed painfully

Nurse 1            : let’s bring her to examination   room 

            After arriving at examination room,   nurse asked to the  patien and her sister to  sit,  then the nurse assessed  the identity of the patient  by assess the information from Rahayu, because at the moment Vera felt lassitude.

Nurse 1              :have a sit please ,

Well   my name is Nurse Ayu and  I’d like to  fill this form by assess  the  identity of older  sister. What is your name ? and What is your younger sister name ?     ( nurse talk to Rahayu )

Rahayu              : My name Rahayu , my Younger Sister name is  Kadek Versilia Suci Rahayu

Nurse 1              : how old is she ? and where do you live ?

Rahayu              : she 16 years old  and we live  at Tukad Balian stree No 56

Nurse 1              : Well   Miss Vera , Would you mind if I ask you some question ?

 Vera                 : no I wouldn’t mind

Nurse 1              :Miss Vera what seem to be bothering you ?

Vera                  :my  head throbbed painfully,  I have a fever and on my  skin look a lot of  red spot

Nurse 1              since when have you felt  that symptoms?

Vera                  : I think I have felt headache since 2 hours ago , I  have felt fever since 5  days ago , and the red spot have appeared since yesterday.

Nurse 1              : So your Complain are    you have felt headache since 2 hours ago may I your pain scale   if on  a scale of 0 to 5 , with  5 being the worst , how would you rate what you feel right now ? in addition , you also said  , you  have felt fever since 5  days ago , and the red spot have appeared since yesterday

Vera                  : yes  nurse ,   those are my complain , I think my pain scale is 3

Nurse 1              : Well Miss   Vera and Miss Rahayu please wait a moment because I’d like to  inform the doctor what   Miss Vera’s Complain .. and for a  few moment the doctor will come here for checking Miss  Vera’s condition

Vera and Rahayu        : yes Nurse …

 In doctor room Nurse   informed Doctor  , that  was the patient that waited at examination room , and then The Nurse also talked to the doctor the complain of the patient

Nurse 1              :  excuse me Doctor

Doctor               : What happen Nurse ?

Nurse 1              : Doctor there is the patient that has waited at examination room , her name of the patient is  Miss Vera , her complain are   she has felt headache since 2 hours ago, she  has felt fever since 5  days ago , and on her skin look a lot of  red spot  that appeared since yesterday

Doctor               : Other  symptom ?

Nurse 1              : she also looks so lassitude

Doctor               :  Well let’s go to examination  room ,

            Doctor arrived at Examination room then she   checked  Cintia’s Condition

Doctor               : your name is written here as Miss Vera , is  that right ?

 Vera                 : yes that right doctor

Doctor               :  Miss   Vera   just now the nurse said that your complain are you have felt headache since 2 hours ago, you   have felt fever since 5  days ago , and the red spot  on your skin have appeared since yesterday. Did you take any medication for it ?

 Vera                 : No I didn’t

Doctor             :  Well Miss  Vera now I’d like to check the condition of your mouth so please open your mouth with tongue far forward (  Vera opened her mouth with tongue far forward) then I’d like to  check your abdoment .

After doctor finished checking Miss Vera’s condition ,  the doctor conveyed the result to Miss Cintya  after that doctor  talked to the nurse,  in order to nurse checked  patient’s vital sign  and also ordered to the nurse for taking blood sample of Miss Vera

Doctor            : Miss  Vera  the results of the examination that I do for you is in your gums looked there was a little bleeding and tongue looks a bit dirty.  And  base on plapation that I  do on your abdomen show your liver can be palpable  and it prove that organ have an infection   And now the nurse is going to check your blood pressure , pulse, respiration , and your temperature . trhe nurse also is going to take your blood for laboratory examination , would you mind if the nurse take your blood sample foe laboratory exemination?

 Vera               : No , I wouldn’t mind

Doctor            : Nurse , please check patient’s  vital sign and please take the blood sample of Miss  Vera for Laboratory Examination  ,  because I’d to check the other patient please tell me the result ,  when you finished  doing examination to Miss Vera

 Nurse 1          : yes Doctor

The Doctor leaved the examination room then the nurse check vital sign of Miss  Vera

Nurse 1            : excuse me Miss Vera , now I am going to check your blood pressure, respiration, pulse, and your  temperature,  I think it’s  spend 5 minute  and also I am going to take your blood sample . before I  am going to check your blood pressure I’d like to ask a question for  you , did you have hypertension before ?

 Vera               : no  I didn’t  have hypertension before. Nurse help me my head still throbbed painfully.  Please tell me how to reduce this pain ?

Nurse 1            : Well for reducing your pain you can relax your   self as able as possible   and we can use distraction and relaxation technique by take a deep breath and breathing out ( a few moment latter) Miss Vera What’s your feeling now?

Vera                :  although it did not significantly reduce the pain that I felt,  but it was effective enough to make me relax and much better than before.

When  The Nurse was  going to check   Vera’s vital sign suddenly Vera’s  Mother arrived at Hospital and she looked so panic  then she asked to Nurse about her daughter condition.

Mother            :  Nurse, how is the condition of my daughter  ?

Nurse  1           : she is still having headache, fever,  and on her skin visible red spots. Excuse me Madam May I have your name ?

Mother            : my name is Chintya

( Rahayu and Chintia made noisy in examination room)

Nurse 1            : Miss Rahyu and Mrs.  Chintya  do you want to wait here or do you want to wait outside  ? if  you want to wait here please be quite .

Mother            :  I think it’s  better if we wait outside 

Mother and younger sister of  Vera leaved examination room and decided to wait outside .   otherwise in examination room Nurse is checking  Vera’s  Vital sign

(nurse was checking Vera’s Vital sign)

A few minute letter when nurse have  finished checking  Vera’s vital sign then nurse tell  Verat he result of that examination

Nurse 1             :  Miss  Vera the result of this examination is your blood pressure is  90/60 mmHg, it’s bellow from the normal rate  and I think  this is causes  you feel your  head throbbed painfully ,    your temperature is 38.9 ^o C    , your  pulse is  88 times/ minute, and your respiration rate is 18 times / minute  and doctor this the result of Miss Vera’s Laboratory

After nurse  finished checking vital sign,  then nurse take  Vera’s  the blood sample  for laboratory examination  when nurse have finished taking   Vera’s  blood sample and then nurse called  Vera’s family  for enter to the examination room.

Nurse 1            : excuse me Mrs. Vera   the examination of your   daughter  have  finished  and you can enter to examination room

Mother            : yes nurse

A few minute letter nurse gave the result of  Vera’s examination to Doctor  at Doctor room, after that doctor   went to examination room for informing the disease that is suffered for by   Vera

Nurse  1           :  Doctor this is the result of Miss Vera’s examination, that is consist of vital sign examination, and laboratory examination on vital sign examination of result is her blood  pressure is  90/60mmHg, her temperature is 38.9 ^o C   , her  pulse is  88 times/ minute, and her respiration rate is 18 times / minute . and doctor this the result of Miss Vera’s Laboratory

( doctor read the result of Miss Vera’s examination )

Doctor             : ok let’s go to examination room

At examination room

Doctor             :  Well I’d like to inform the disease that is suffered from miss  Vera , before that may  I know which one is the parent of Miss  Vera ?

Mother            : I am parent of  Vera

Doctor             : Madam your daughter suffer from dengue hemorrhagic fever this disease caused by virus that enter to Aedes aegypti., to prevent something undesirable, your daughter should be hospitalized,   would you mind if your daughter should be hospitalized?

Mother            : No doctor I wouldn’t mind

Doctor             : Well if you agree about it please sigh the informed consent document which means that you agree with the actions that we do on your daughter.

Mother            : Ok doctor I am going to sigh it

After  Vera’s mother sigh informed consent document then doctor told to nurse for calling  admission  Department of  Rose ward to register  Vera    for hospitalization  then

Doctor             : Nurse please call the rose ward to register Miss  Vera    for hospitalization there

Nurse 1            : yes doctor

(kring-kring )

Nurse 2           : Admission Department of Rose Ward  , I’m nurse Ayu Dwimatari Can I help you ?

Nurse 1            : is there any room on Rose ward available  ?

Nurse 2           : yes here there are some  room that available, I’m sorry may I have your name?

Nurse 1                        : I’m nurse ayu   from polyclinic  I ’d like to register  patient suffers from dengue hemorrhagic fever for hospitalization in Rose ward . 

Nurse 2           : Ok nurse ayu ,  may I have the patient’s  name ?

Nurse 1           : her name is Vera

Nurse 2           : Well nurse Ayu , now  Miss Vera have been register here , and  you can take along your patient  to Rose Ward in ten minute because we are going to prepare all the thing for the   patient

Nurse 1           : ok thanks you nurse .

            After   nurse registered  Vera to rose ward doctor give prescription that have to change in Pharmacy  by Vera’s family 

Doctor             : excuse me madam I want you to change this prescription in pharmacy.   by the way did you daughter have any allergic with certain medicine ?

Mother            : No doctor she didn’t has any allergic with certain medicine   (  Vera’s Mother  took the prescription)  and  I am going to change this prescription

When  Vera’s Mother wanted to change the prescription to the pharmacy , suddenly  Vera’s younger sister  talked to her mother that  she can’t stay at hospital anymore because she have to prepare  all the thing for  doing study tour tomorrow

Rahyu              : Mom  I can’t wait in hospital anymore because I want to prepare all the thing for  doing study tour tomorrow

Mother          : Ok honey you can leaved this hospital, take care honey and I’m sorry I can’t help you prepare all the thing for doing study tour

A few minutes   later   Vera’s   Mother arrived at pharmacy and then she change the prescription

Mother            : excuse me I’d to change this prescription

Pharmacist       : Ok madam please wait a moment (pharmacist took the prescription )

For a moment later

Pharmacist       : Madam this is the medicines , the medicine consist of   RL,   paracetamol,  Imbos Forces,  and Lemeson .   and please bring this  medicine to the nurse that  take care of  patient at Rose Ward today. And this is the receipt    do you have any question Madam ?

Mother            : Ok I’m going to bring it to the nurse that take care of the patient in Rose Ward to , and  I think I don’t have  any question  . and thanks for  information

Pharmacist       : you’re welcome

After   Vera’s mother changed   the prescription in pharmacy then she come back to polyclinic  . for take along her daughter to rose ward

At Rose Ward

Nurse 1            : excuse me I’m nurse Ayu from polyclinic  , I’d to take along the patient for hospitalization here

Nurse 2            : may I have the name of patient ?

Nurse 1            : her name is Miss   Vera

Nurse 2            : ooooo, Miss  Vera ,  your room have been served ,  your room’s number is 34 , Well nurse Ayu  did the patient take any medicine in policlinic ?

Nurse 1            Not yet , Nurse this is the therapy that is given for the patient , and you have to continue give  the therapy to the patient

Nurse  2           : yes nurse Ayu

Nurse 1            : so that all about the instruction that is given by doctor, so I have to go to polyclinic now … see you…

After   Nurse 1 leaved the Rose ward  Nurse 2 take along Vera to her room .

Nurse 2            : Miss Vera this is your room  and  you  can take a rest  here,  I’m nurse Ayu Dwimantari , Well Miss Vera now I have to visit the other patient if need helping please press the call button and the nurse will help you .  See you and get Well soon

Vera                : thanks Nurse

After 9 days  Vera have been hospitalization her condition become better than before , she no longer felt headache, her body temperature began to decline and the red spots on her skin began to disappear because of that doctor allowed Vera  for return to home . and Nurse inform to Vera that she could go  back to home

( At Vera’s Room, )

Nurse 2            : Good morning Miss Vera how are you to day ?

Vera                            : I’m fine nurse  ,   my condition is better than before , I  no longer felt headache, her  fever and the red spots on my  skin began to disappear

Nurse 2           : Well Miss Vera  I have good news for you ,  Doctor allow you to return to home today  but you have to control your condition here  for once a week  and you should plenty of rest, drink much water  and take your medicine  that will be given by Doctor  regularly 

Vera                : really ?  if  that’s right  I will do your suggestion

Mother            : Nurse are you sure ?

Nurse  2          : yes  , and for you Madam please change this prescription at pharmacy  and you can pay the bill of hospitalization  at the cashier

Mother            : yes nurse

Nurse  2          : well that all the information and  and now I have to visit the other patient

After Nurse  said that Vera could leave hospital today she and her mother were so happy  when the nurse leave Vera’s room  then Vera’s mother  directly  went to cashier  for paying the bill of hospitalization and changing the prescription at pharmacy

  At pharmacy

Mother            : excuse me I’d to change this prescription ( gave the prescription to pharmacist )

Pharmacist      :  please wait for a moment madam

 For a moment latter

Pharmacist      : madam this is the medicine  , the medicine consist of paracetamol,  Imbos Forces,  and Lemeson .  paracetamol is used  for  Lowering a high temperature (fever).  And Please take one tablet 3 times a day .   For Imbos Forces it’s used for increasing  antibodies , because DHF is caused by virus so that our body need good antibodies system please take one tablet  per  day  and the last one is lemeson it’s used for coping with inflammation in liver please take one tablet  twice a day . do you have any question Madam ?

Mother                :   no , I don’t  any have question

Pharmacist      :   may I know how is sick  madam?

Mother            : my daughter

Pharmacist      : please remember to your daughter   that she have to drink a lot water ,  and please take this medicine until use up  and the this is the receipt

Mother            : ok thanks for your   information

Pharmacist      : you’re welcome

 Finally After Vera’s Mother changed the prescription then Vera and her Mother is return back to home and this is mean the end of our drama
